Is this a Biomet 3i Nanotite Implant?
This patient came to my office complaining about the implant being mobile. The implant was placed about 15 years ago by a different dental practitioner. I took a radiograph and verified that the problem is the abutment-crown interface screw loosening. Looks to me like a BIOMET 3i NanoTite implant with a screw retained crown. However I would like to ask for your feedback as well to make sure I am on the right path. Other tips to identify this implant are also greatly appreciated. Thanks for your help in advance.

This is not a biomet 3i nanotite implant. The nanotite has only been out less than 5 or 6 years, and the retaining screw threads are not like 3i's, these stick out wider than the screw shaft. Not sure who made this implant, maybe check out whatimplantisthat.com

Similar, but the abutment screw appears to be Nobel (Uni-grip Abutment Screw) circa 2001. My guess is that this is a Nobel ex hex implant with a Ceradapt or similar abutment. The screw appears to be all the way down but I'm suspicious that the ceramic crown is sliding off the abutment. Test it by grabbing the crown with rubber tip pliers and rotate-pull.
The 3i implant has an uninterrupted transition to the flared platform; this has a step which makes me believe it is a flared abutment instead.

The patient came in today and I removed the crown with cotton pliers as Mark mentioned. The implant is a Nobel ex hexagon. The abutment looked stable however I tried to examine the head of the screw just in case there is an issue in the future with the abutment. It looked like it was square and not hexagonal therefore it is probably a cera one abutment. I positioned also the crown so that it will adapt fully to the platform of the abutment and cemented it permanently.
So we have nobel ex hex implant, cera one abutment, nobel screw, all ceramic crown cement retained.
