Broken screw straumann implant: recommendations?

I have a patient who has a broken healing abutment (yes healing-not sure how someone did this)… patient also has part of the screw inside the implant. I have tried to use the Straumann rescue kit but it did not work. Another periodontist tried with ultrasconics but still could not remove the screw. What can I do to get the remaining threads out? I have place a modified healing abutment in place for now ( I cut some of the threads of a healing abutment so It would fit). The implant on the xray is the one on the right. Any recommendations how to proceed?

Contact the endodontist you work for assistance. Magnification is the key. If it wont move with ultrasonics, drilling down the center of the abutment screw and either slotting or sectioning the fragment may be the only option. Not a hopeless situation yet.

Take proper history from the patient, did he/she have restoration before on this implant then the screw broken? it might be covered by his treating dentist waiting for other treatment? or he never had any replacement yet and the broken piece is from the healing abutment Is the broken piece of screw movable? you check it by pointy probe If the broken piece mobile you still could remove it easily with a lots of patience. Take your time and use a lots of irrigation intermittently (any mouth wash) and unscrew the broken piece carefully with probe. try don't use ultrasonic scalar because sometimes it screw it deeply but you could try with avoidance of touching the internal threads by touching the broken piece in the center. The procedure could take only 30 minutes If the broken piece not moving at all after all this procedure consider total implant removal using implant removal kit or triphine Good luck

The threads coronal to the screw fragment may be mashed not allowing anything to be backed out past them. Talk to your straumann rep to get a rethreading tool slowly advance it into the implant till it bottoms out run it back and forth a few times to retrace the threads then retry backing out the screw piece with an instrument

Ultrasonic rarely helps in those cases. The screw removal kit has to be used by a practitioner that has experience in removing broken screws, they usually do the job in almost all cases but you have to be persistent and patient. It is very essential that the case gets to the right person immediately before multiple people try to remove the screw. Talk to your Prosthodontist and see if he has any experience in this field ... Magnification is key but an Endodontist with no knowledge of screw removal might hurt the case more than help. Make sure you inform the patient that you are trying to help and that possibly the implant might need to be removed " Sign a consent form ". Rethreading might help ... But try to send the patient to a practitioner with prior experience with similar cases before you try to access the head of the broken screw of do anything else.

Never saw a broken implant screw's head other than in patient's mouth.youneed to remove what's left of it.For me, ultrasonics worked fine in ALL cases of broken screw.
