Dental implant placement in patient taking methotrexate?
I have a 50-year old patient suffering from arthritis for 20-years. She has been taking methotrexate for 20-years, initially IV, currently orally once a month. She is missing a mandibular first molar which she would like replaced with an implant. Radiographically bone looks sound. Are there any contraindications to me installing an implant in her first molar site?

Talk to the MD who is treating the patient. What does the bone density and occlusion look like?

Methotrexate (MTX) is an anti-metabolite used in rheumatology and oncology. In high-dose MTX (30 mg/kg) impaired bone healing process.

Last year, I had a patient who lost #9 due to a vertical fracture. After a literature search which found no credible studies concerning implant placement in Rheumatoid Arthritis Patients on Methotrexate. Following the extraction with 70/30 mix of allogenic bone grafting with a collagen membrane, and allowing for one year healing, Re-entry showed poor mineralization and bone formation. We decided to restore with a bridge instead.

Would implant placement on people on methotrexate lead to failure?

I've treated countless patients with implants on Methotrexate. Those that had healing problems typically had co-morbid factors.
