Dental Implant Temporization Problems
A patient who required multiple dental implants is four months into the maxillary case and did not want to wear anything removable. I thought I was such a smart guy by staging the case using some remaining natural teeth to support the fixed temporary. Well the patient came in with his heat processed temporaries broken off and in pieces. The teeth are down to the gum line. I thought about IMTEC transitionals but I am not sure they will stand up to the abuse. Only two months left and I am thinking of pushing the time but it was type two and three bone. Any thoughts? I cemented the temps on with Panavia until Thursday.

Because of the problem you are describing, I only use Zimmer internal attachment implants (Tapered Screw-Vent and Advent). They have a Morris taper that solidly locks the abutment to the implant when it is torqued down. I have never had one come loose. I have begun to use Innova Endopores again also because they have an internal attached implant that has a Morris taper.
When I was using external attachments or any abutment that was only held in place by the screw, I had the nightmares you are describing. Check into the Zimmer and Endopore implants.

I too like the Endopore with the internal Morris taper. But these are cases where the teeth have not held up while the temps are on. The question comes into play of using implants as a temporary solution.

I have been in similar situations and have been very happy with the IMTEC mini transitionals. If you are sure about only needing them as transitionals, you would want the predecessor unetched dentatus type, since the surface preparation of the IMTEC gives them a high degree of integration if you leave them very long. It is amazing to the patients to have MTImplants placed and not require pain pills ... this helps when you have to add this un-anticipated step. I get great durability with Connect fiber reinforced Bis-Acryl resin temps, sealed with Biscover.

I am very pleased with IMTEC and am always surprised at the integration. But I always use them in conjection with conventional implants and if they stay I am happy but if not I have the other root form. I always present them as immediate function....no denture flop but they may not stay. Great idea to connect them for stability.

Dear Dr. Hoos:
The provisionals you have used...are they re-inforced with a properly designed metal casting? (not a wire). My practice is devoted almost completely to this type of work, and very rarly do I get fractures of properly desined metal re-inforced provisionals. They can hold easily for two yeares on a minimal ****** of teeth, and temporary implants work incredibly well in these situation. The type of implant in this case is completely irrelevalnt. As a matter of fact, most practitioners who restore such cases on a routine basis prefer screw type restorations on external hex implants. This traditional, proven design allows for most flexibility surgically and prosthetically. (the labs understand it much better, as well). There is quite a bit of literature published on metal re-inforced provisionals, especially from the New York University College of Dentistry--Post-Graduate department of Implant Dentistry. Check them out... Good luck with your patient.

Dear Dr Kaufman
I have gone to the casting method but I am very interested (and our other readers) in the information about long term provisionals and maybe you could get our group the link for the information......you have nothing else to do :)
As far as external hex verses internal hex, I find the internal so much easier because of the positive seat and the lack of screw loosening.
I understand that a loosening screw could be indication of a problem and better to have a screw break then loose an implant.

Drs. Camm & Hoos both make reference to a "Morris" taper but the actual reference should be a Morse taper, an internal friction-only connection between abutment & implant body, first introduced by Dr. Tom Driscoll for his own original implant system, which is now marketed commercially as the BICON implant system. What a pleasure to have Dr Sendax contribute to this thread. The use of the soft liner is a great idea with IMTEC. This is most important when the occlusion is suspect. Thanks for the Morse clarification and thank you for innovating IMTEC for us and our patients.

i am having 4 teeth removed, synthetic bone put in then in 2 months the implants(unloaded) put in. Why wont my oral surgon put me to sleep? Gonna take Valium but dont know if that will keep me calm.

I am looking for somewhere on line to find some information on any problems with veeners. I am having problems and I hate any kind of confrontation. I have never been happy with the way my venners look,taste,or feel. I just figured it was me,had them checked told they were fine. For a long time I have had a very sore and very stiff neck and throat. Finally had some lymph drainage massage on it and my mouth and teeth feel like liquid old blood is seeping. My mouth is very red it hurts and I feel sick. Any suggestions Thanks Marnie
