Maybe an old screwvent from the old corevent company (now zimmer).
In any case, when you compare it to the adjacent implant, it looks like the prosthetic platform could be broken.
You have two options.
1- Drill through the adjacent crown and unscrew the abutment to look at the actual abutment connection and try to find something that matches it, or
2- Remove the implant and replace it with a new one.
I would tell you to go to the whatimplantisthat website, to identify it, but I don't think they have data on it.

mAYBE AN OSSEOTITE Certain. A more perpendicular x-ray would help.

I would agree with Carlos it appears that the fixture has fractured at the coronal and internally looks like it may be distorted. Is it possible to take an intraoral pic and post it showing the top of the implant?

I agree with Carlos. It looks like a Corevent (now Zimmer) implant.

I send these questions to Abutments International. Contact is Darwin (he probably has a last name also). I have sent him x-rays and failed prosthetic components for identification. They will have the prosthetic components that you will need.
The implant is fine, the x-ray is not perfectly perpendicular. Bone looks great for ten years!

I think that Zimmer will also ID any implant if you send them a radiograph??

Radiographically the bone seems to be in good shape. If you don't get the prosthetic attachments for this implant, still you can rehabilitate the patient with post n core kind of attachment. The screw diameter for any implant ranges from 1.4mm to 1.8 mm with the lenght ranging from 6mm to 8mm so a post for the same can be casted with the hex for the precision and will act as anti rotation for the crown. This can be cemented after checking the precise fit of the post with a radiograph.Subsiquently a crown can be fabricated and cemented.
I have done few cases like that by far the results are good
