The patient presented with an implant that was 30 years old. Can anyone identify this implant?

Its a branemark clone

With high probability, It appears to be an OIC external hex Implant, "osteo implant corporation".
A distinguishing feature of this implant was the indentation at it's Apex. The implant was mounted from it's Apex to it's vial cap. This implant was Life-Core external hex clone. It's universal size was duplicate of branemark but it's wide platform had a wider abutment screw. Unfortunately neither OIC nor Life-Core exist any longer. There are companies out there that still make non original parts. A good stop for you would be Titan implants. I hope this info can be of help.

Off the subject,
But people have been tilting implants to avoid the sinus (and other anatomic structures) for a long time!!!
This surgeon had the insight to keep things easy for his patient, even 30 years ago when much about implant dentistry was a mystery.
I would like to shake this guy's (or girl's) hand!

Another case of guess the implant. I've now retired but it was my practice to give every implant patient a laminated card with all the details - date of placement, name and address of supplier, name and dimensions of implant, catalogue number of fixture, abutment screw and abutment, etc. In my opinion this should be standard protocol and any surgeon failing to do so is frankly negligent. In the UK we have a mandatory National Joint Registry for prosthetic joints and there are at least 23 national joint registries, either voluntary or mandatory, elsewhere. They have proved to be hugely important sources of data for evidence based decision making. This is sadly lacking throughout the dental world and of course implants are no exception, which is why there are so many implants floating about with absolutely no evidence base whatever for their design.

The recent comment about Experience is a collection of errors was on point love it!! Be carful with densa burs I had 3 initial failures in reverse last year and made it my worst in the past 27
