Implant #8 area: suggestions grafting?

My patient desires an implant for #8. Facial plate is present, but there is obvious defect (canal) on lingual. Suggestions on bone grafting the lingual aspect here? Release tissue, well enough with releasing incisions, decorticate, and place particulate bone graft and membrane? Thank you in advance!

Wow! Tough case indeed. Many have disected the NP canal and grafted bone right into the site. I personally have never done that and not a big fan of that. I would consider instead Grafting the buccal via Khoury technique or any other predictable technique and widen it buccally instead and place implant slightly more buccal and use custom abutment. I also would consider a narrow implant here (like a 3.0 or 3.5mm). As mentioned, Bridge may not be a bad option either, pending what the patient elects. Good luck and keep us posted!
