Intra-Lock Implant System to Replace Maxillary Lateral Incisors?

Dr. H. asks:
Has anyone used the Intra-Lock mini implant system to replace maxillary lateral incisors in a space too small for conventional implants? Their 3.3mm diameter implant seems ideally suited for that kind of situation. I have never used their system and want to get some input from users. Is this a reliable implant system for this kind of situation? I was considering using these implants in this case because the patient is young and the adjacent canine and central incisor are virgin teeth so I really do not want to do a bridge. It would also be difficult to match the central incisor and I would have to do the other central incisor as well.

I just placed a 2.0 mm MDL Intralock inthe position of #25 last week. Surgically it was pretty simple process. I did not load it immediately however. Instead I provisionalized with a bonded pontic to the adjacent teeth making sure that implant was not engaged. Very important to let the pt that this is an unconventional approach and that success rate is lower with minis. I also strongly urge you to disregard the manufacturers recommendation of using a flapless technique and to actually flap and ensure you are in good sound bone.

I have used the Intra Locks for maxillary laterals. Even the small diameter (two different diameters) seem to work well. The MIDI is 3.0 and offers three abutments, straight, angled and castable. The snap on/cementable abutments are also available for the smaller diameter implants.

i had a similar case, i used midi implant of osteocare, and it worked just fine. use the flapless technique only if you are expert enough, and remember to keep a look of your direction while inserting the implant. best of luck

I place INtralock implants very frequently in my practice. Intra Lock has research showing there ossean surface has bone healing at 6 weeks that will need 250NCM of torque to reverse the implant. THat aside I have placed the MDL and MDI for fixed and removable prosthesis. I have immeidately loaded almost every one. My success rate is approx 90-95%. BUt they are great implants (due to the available prosthetics) for small m-d spaces and for times when the patient cannot afford or does not want bone grafting.

I would strongly advice against 3mm or less diameter implants that have a screwed in abutment in these sites. you will be risking failure due to abutment fracture since the screw hole is going to make the preped abutment thin and susceptable to fracture under load. your best bet is a 3mm ONE PIECE implant

I am using this system very frequently, It sounds very well. you can also try the MILO system of intrtalock (3 mm diameter). it works very well in this situation. my success rate is above 95%. best lock

Both Astra and Dentsply have a 3.0mm diameter implant. I disagree with a previous commenter that two piece implants should not be used. It is very difficult to prep a one piece implant and provide an esthetic restoration. In an essentially non functional tooth like a lateral incisor I'd go with a proven system and company like Astra or Dentsply. The two piece system gives flexibility in positioning the finish line for optimal esthetics.

The 3.0 mm Intra-Lock MILO implant is, in fact, a 2 piece system. It has a patented ball top assmbly with a cement on abutment. FDA fatigue tests show that these implants have dramatically higher strength than internal attachment implants of the same diameter. The additional advantage of the OSSEAN surface is that it gives you a 500% greater bone boding strength at one week than the other two leading surfaces (nanotite and osseospeed - PUBLISHED. Great for immediate provisionalization and immediate load cases.

i have used imtec and intra-lock implants for these areas since 2002, and have hundreds in place, from 1.8mm to the newer milo 3.0. not one has fractured under load in the lateral incisor position, although we are careful with loading, and do not immediate load all of them. lateral locclusal forces, particularly immediate side shift, would kill em, so understand your occlusion when using smaller diameter implants! initial torque is important, but other factors, such as premaxillary bone angle, how robust the dentition is in general, and whether parafunction can get to the tooth, are factored in. our success rate is over 99% over 6 years on lateral incisors. use with caution on other areas. i have been using the ossean coated intra-locks for the past 18 months or so, and they are very good! in patients with small teeth in general, where most systems are difficult to manage, we have been successful with one piece, single stage implants, where the restored tooth/ implant surface area is similar to that of large dentitions/ traditional implants, great alternative to treating these difficult patients. the gang at intra-lock is very helpful getting started with these implants.

Hi, I am a patient; I have lost two front—top— teeth; I had a surgical procedure and had 2 fixed implant with screws. Now it is time to get prosthesis (teeth) to seat on the implant. Hi, I have been placing IntraLock MDL and MILO implants for nearly 2 years and have been placing mini implants for almost 9 years. I have placed over 7000 mini implants and about 1/2 for crown and bridge use. We have over 98% success with mini's for crown and bridge. I encourage all dentists to start doing mini implants. I have also placed Intra-Lock minis to replace maxillary lateral incisors. Usually 2.5 x 15 or 2.5 x 18mm. Intra-Lock also offers a 3.0 MILO one piece implant that uses the same driver system as the minis. I have three cases where we replaced the missing laterals in one visit with the PFM crowns and surgical stent from Shatkin FIRST labs. Best of luck with your case.
