Maxillary Overdenture: Ideal Location for the Four Implants?

Dr. S. asks:

I have treatment planned a patient for a maxillary overdenture retained by four dental implants. What are the ideal locations for installing the implants? I was thinking that the sites that would provide the most retention would be #3,6, 11 and 14 [maxillary right first molar, right canine, left canine and left first molar; 16, 13, 23, 26]. I have seen these sites recommended in textbooks and in lectures. Is this correct?

Implant placement even if prosthetically driven is dictated by the anatomy and available bone. Do you have a radiograph to share? Too many question marks exist...how long ago were the teeth extracted? Are the sinuses pnuematicized? For example.

I have done a lot of the bar over dnetures. Infact my lab thinks there is no one in my area that does more. I ideally place them in teh #4,#7,#10,#13 areas but I would take a study model of the patient adn place the implants in locations tht will not cause an anterior cantilever adn that you get a great AP Spread. Also, know what you are opposing, and if the patient has a strong jaw muscle( Short Angle). This may warrent more implants

a maxillary overdenture is a difficult restoration to do. Is this your first? If so, get the help of a prosthodontist. You need mounted diagnostic casts to evaluate the interarch space. There needs to be room for abutments, the bar, attachments and denture teeth. You don't want the implants placed them later find out you don't have room. If the interarch space is tight, then a crown and bridge restoration is more appropriate with 6-8 implants or have your surgeon do an alveolectomy first. Either way, this is not an easy restoration. It needs good presurgical planning to evaluate bone shape and size, anticipated type of restoration and tooth positions and implant angulation and position.

If you do not know the answer to this then the BEST location for the implants is them being placed in a specialists office. Good luck.

I have done many maxillary overdentures.First I agree with teeth # 4/7/10/13 placement site.It is very very mandatory to find interarch distance available.I believe Bar is only required in maxillary.O.D.compared to free standing.I also feel that Hybrid shall be preferred over the O.D.in maxillary planning.

I prefer use minimume (at least) 5 implants in upper jaw, in sites second premolars, canins and incisal to prevent of rocking and have great A.P.

I use 10 mini implants with housings. Very little space needed. Easy for dentist and/or patient to remove if repairs needed.

I don't see any major differences between 4 locators vs a Bar retained OD, except, with increased VOD, yes a bar will give better lateral support, and in an ideal situation, 4 locators will do very well,and easier for the patient. I believe if the patient is welling to go for 4 implants supported OD, is better to switch to All-on 4 concept, as a fixed vr removable with almost close price range. also if you don't wannna lose the patient in the future, don't do mini's
