Measuring and Assessing the Zone of Keratinized Tissue Around Potential Implant Sites?

Anon. asks:
I have not been able to find specific guidelines for measuring and assessing the zone of keratinized tissue around potential implant sites. This area of per-operative diagnosis and treatment planning is not as well developed as some other areas. This makes it difficult to treatment plan cases and avoid complications. In courses I have attended various lecturers have recommended doing various grafting procedures to increase the zone of keratinized tissue before the surgical placement, during the surgical placement or after the surgical placement. I have a patient who needs to have the zone of keratinized tissue increased around the mandibular first and second molars. What are the best techniques to do this?

The free gingival graft and the subepithelial connective tissue graft are the most recommended procedures. Some clinicians use the acellular dermal grafts as an option. Grafting prior to implants or during exposure are choices you have to make, each one with advantages and disadvantages.
