Patient from Brazil: which brand of dental implants is this?

I have a new patient who had implants installed in Brazil. The implant crown on #30 [mandibular right first molar; 46] is loose and I am not sure what treatment will be needed. He cannot remember which doctor placed them or in which clinic they were placed. He does not have any information on the brand of implant or its dimensions. I would appreciate it if anyone recognizes this implant brand and can help me with its identification.

Looks like a 3i External Hex Wide connection compatible implant. Drill a hole to the abutment, change the screw to a new RP Branemark screw or 3i wide screw, check interproximal contacts, torque it to 35N.cm, use monopaque resin to cover, check occlusion and that is it.

Is the abutment fully seated?

It can be a Neodent ALVIM 4.3 or 5.0

It does not really matter to know the brand. It appears to be only a screw loosening here. Is the crown screw retained or cemented? 1- If screw retained, piece of cake. Unscrew, get a new screw and torque it down. 2- If cemented, you would be lucky to get the screw access hole thru the occlusal surface. More likely that will end up redoing the abutment + the crown. Once you'll those parts in your hands you will asses what kind of hex, and what kind of screw. As there is nothing "original" Made in Brazil, you will recognize one the standards that have been used....

We've been placing a small ball of Teflon tape in the screw head before we seal the access hole with composite. Less bacteria than cotton but still allows future access.

Definitely try to drill an access hole and tighten in a NEW gold screw. Once a screw is loaded we all know it deforms and them continues to loose again and again. BTW it came loose because #3 supra erupted when the peri apical formed. Make sure you clear that up ASAP.

Looking at mechanical data, use any screw but NO GOLD screw. Too ductile and subject either to loosening or breakage.... Gold plated OK. There are better screw coating around to ensure optimal preload.

looks like a 3i Osseotite external hex to me. I agree with replacement of screw with correct torque setting and sealing of screw access with teflon tape. Consider reviewing occlusion on this implant as these implant restorations are more susceptible to flew on lateral movements

It seems to be 2 types od Neodent implants: Alvin TI (external exagon), 4.3x13mm Alvin II Plus (internal exagon), one 5.0x10mm, the other 4.3x16mm

TMG, that was an interesting response. I have always advised using gold screws based on literature/data, albeit sponsored by implant companies. Tell us more, who makes these gold plated screws and are there any studies?
