Patient with Fractured Screws: Any Recommendations?

Dr. O asks:
This patient came in with a fractured implant retained bridge in the lower right. The mesial screw was completely out and the distal screw had fractured. I managed to retrieve the distal screw. The implants were placed by a German dentist in Spain but he has since disappeared….implant heads are very damaged, but I am trying to find suitable screws to get the patient out of trouble whilst we plan the replacement implants The implants have a coarse thread and are very narrow. The lady was told by the dentist that they were Straumann implants!! Does anyone out there know what this is and where I can order replacement screws? The first radiograph shows Biomet 3i Gold-Tite screws in them, which are apparently too large. What are your recommendations and thoughts?

I work in spain and know your pain! Do the goldtights thread ok but bump stop at the end of the thread? I used to use 3i but really cant remember much detail im afraid, however I do seem to remember that there are some temp abutments which come with a non coated retaining screw where the threads go all the way to the head. I think that it is a standard pitch and diamiter, and a number of manufacturors may use the same and have something you can use. I have a feeling that Ankylos may use the same pitch in some of their screw retained prosthetic elements (again may be making that up). Looks very similar to the screw for occlusal crown retention on a standard ankylos abutment, but that's a bit of a punt in the dark! All the best, Mike

Why would someone place such narrow implants in the posterior, and why would we splint them? This pt. lost their posterior teeth once. Now he/she is on the verge of loosing these. I wasn't there and don't know enough to be critical, but this seems to be an avoidable mess! Bvinci

Baker here in Spain people advertise implants for €395 fully restored. People ask me why we are so much more expensive all the time. Fixed Full arches for less than 3K we see this more and more sadly.

So mike, are you saying that 2500 an implant is too much? That's not actual charge , but after ct ,graft ,gtr,and anesthesia fees,it can actually be more. West coast is even more expensive. Are we price fixing? Bv

Have you investigated BTI implants which are made in Spain? Maybe they have your hardware?

Those appear to be General Implants out of Germany http://www.general-implants.com/shop/ my suggestion is get a stock abutment for each and lute them to the implant using a resin cement and treat as a one piece implant. If you can not get parts then using the 3i screws and GC pattern resin make a castable core have it cast then at insertion place some resin cement on the connector and place the screw thru the core.
