Implant positioned too far from tooth# 19?

I got an implant installed at the #18 site a week ago. The implant looks as if it is positioned too far from tooth # 19. Looks like it will need a crown a lot wider than natural tooth (at least 40% wider to sit on center of implant so as to distribute forces evenly) . Or else there will have a cantilever effect. Do you have any advice you can provide? What are the potential solutions? Thanks a lot in advance.

You aren’t accounting for the custom abutment.

That’s not terribly bad (trust me I’ve seen worse) I would do this one screw retained., The lab should make this the width of a normal molar. Should be fine.

The distal root of 19 forced it to be placed more distally. I have run into that before. It should be fine.

I don't think it's too bad. It looks like the implant is positioned in the middle of where the two roots of the natural tooth used to exist.

It's actually very well placed. This site behind a full size first molar is not easy to visualize. And the drills keep hitting the marginal ridge of the preceding molar as well. This is actually a good placement. A nice screw-retained crown will give you years of service. The trans-mucosal healing abutment placed concurrently makes it appear as the bone-level implant was placed too far out of bone, but on close inspection the abutment and implant look perfectly positioned. Buy your dentist a good bottle of Scotch once the crown is made.

Ummm .......# 18, #19, cantilever?? I dont believe this to be a patient post, at all.

Implant looks pretty fine. You have to be very careful about cleaning and maintenance, ask your dentist about it. Besides, that X-ray shows a periodontal disease in the anterior lower teeth, get a deep cleaning and take care of your oral hygiene.

Well placed, shouldn’t be an issue. Make sure you have someone look at your lower front teeth to address the loss of bone. Also it’s probably time to think about those wisdom teeth on your right side.

I think your dentist was grabbing some good bone on distal aspect. With custom abutment, this will be a very successful restoration.
