Sensitivity while drilling to place implants?

Last Updated: Jul 31, 2016

I have a patient who is congenitally missing her two mandibular central incisors. Recently I placed two 11.5mm long implants in both sites. It appeared to me that there was no chance for nerve involvement and that the nerve was far below in the body of the mandible. However, while I was drilling with the 1.8mm osteotomy drill, the patient felt sensitivity, but felt nothing when I installed both implants. The implants are 1-piece design. Why did the patient feel discomfort when I was drilling the osteotomy? I admit I am a beginner in implants and do not understand this.

In order to help you and answer your question you need to provide a little more information. Did you anesthetize the area with infiltration on the labial, lingual or both. Did you use a mandibular block? If so, did you supplement the anesthesia with labial and lingual infiltrations? Did you use high or low rpm's? And did you use irrigation as coolant while drilling? Thanks,

I anaesthetised the area with labial and lingual local infiltrations only, and used 1200 rpm for drilling with saline as irrigant. Thanks for the reply.

There is plenty of cervical innervation in that region.

does this mean I might have had drilled into a nerve or something?

Mandibular central incisor region may sometimes have cross innervation from opposite side also. Sensitivity while drilling could have been because of that also.
