Soft silicon liner: how long after implant surgery should I wait?
I have a female patient who presented with maxillary and mandibular complete dentures. She wanted a more stable and retentive mandibular denture. I installed 4 implants in her mandible 4 weeks ago and have treatment planned for an implant retained overdenture. How many weeks now should I wait to reline her mandibular denture with a soft silicon liner? I plan to make final impressions for the overdenture in 2 1/2 months.

why do you wish to reline with soft liner?
why not just relieve the denture around the implants wait to integrate then put on locators?

In order to avoid pressure on recently inserted implants, I think soft liner is must. My concern is, when to do relining- a week after surgery, two weeks or a month.

If that's the case immediately after surgery and watch the healing for any imbalances or sore spots. Viscogel works well if it is a reasonably fitting denture to start. If if is an old poorly fitting denture I woul have taken it away for six weeks or have remade it prior to surgery. Trying to retrofit a poorly fitting, not poorly retained, denture may cause problems with bone resorption during integration. Sort of like a flipper over an implant or bone graft, not wise. Good luck only you can determine if the fit is reasonable and the implants are protected. It' s not like waiting for the sore spots after an immediate denture! You can have the implants at risk.

I would use Viscogel straightaway on the day of surgery, and have the patient return 1-2 weekly to change the viscogel over when it starts getting hard. This way, there will always be a soft cushion underneath the denture that minimises the force transfer to the implants while they are integrating. My 2 cents.

Picking up the implants with your final denture impression and having the attachments processed into the new denture is superior to a retrofit. If you feel you need a soft liner now for stability, retention, tissue conditioning, etc. you can do it at any time. Just be sure and relieve it over the implants so no pressure during integration.
