Trinon Q-Implants GIP for Atrophied Mandible: Any Tips?
Dr. C. asks:
I have a patient with a severely atrophied mandible. I would like to place Trinon Q-Implants GIP which are recommended for the atrophied mandible. The implant fixtures are short and hollow. I have never used these before. I would appreciate some advice from those of you who have experience with this implant system. What recommendations do you have for surgical placement?

Short implants provide an alternative to avoid bonegraft. But the reality is that some patient did not tolerate their dentures with short implants. I did not use Trinon Q-Implants GIP, but I removed Biocon implants in a patient that not tolerate his denture. Other possible problem could be mandible fracture.
If the patient has a severe resorption and is looking for long term restoration the best treatments is bonegraft. The most succesful techniques are:
1) Tent pole technique by Dr. Robert Marx and Dr. Thomas Schallemburger (JOMS), the cons is a omega submental scar.
2) Mesh technique by Dr. Patrick Louis (JOMS), the cons exposure of the mesh but if is a delayed exposure it will form a pseudoperioteum below the mesh. The better mesh are preform mesh by Osteomed, but their are expensive (1k per quadrants). The other cons is the removal of the mesh before implants placements.
Do a good case selection before implants placement. Good luck!

I have placed implants in several cases with severely atrophied mandible. Most important is the size selection and location. You may place any implant configuration(root forms). Please note that the mandible at this stage has thick cortical bone and almost no alveolar bone pattern (D4). The implant will anchor on the crestal cortical bone and 50 % on the cortical bone on the base of the mandible. Never place the implant tight (high torque) you may fracture the jaw. There is least amount of blood supply in the cortical bone. The receptor sight be exact size in diameter as indicated by the company - you will do well
