Use of Chlorhexidine 5% For Implants?
Can I use chlorhexidine 5% to disinfect healing caps before placement? Will this help to reduce pathogenic bacteria in the microgap between implant fixture and abutment connection? Can I use it to disinfect the surfaces of implants with peri-implantitis? Is this too concentrated to apply to the surface of the implants and will this cause any damage to tissue or is it safe in this concentration? Will chlorhexidine 5% damage the implant fixture surface?

That won't make any difference. Save yourself the expense. How long do you think until the effect of CHX would be gone and germs would colonize your healing screw or implant?
Body will take care of it.

Well that is true but it is after all a recognised anti microbial.
As an antimicrobial, it is used for skin applications at 5% but is the safe for mucous membranes.
There are no articles on concentration and toxicity of chlorhexidine.

without reviewing literature and using infor I have learned and used I feel the following is accurate:
1. CHx for oral use is .125% and I didn't know 5% is available but would not be a FDA use.
2. A CHx soaking would be fine in 5%, but why not autoclave, whether new or used.
3. I use .125% as a rinse when I remove a cover screw, healing abutment or reg. abutment but this is anti septic at best.
4. I use Neosporin on the implant platform prior to seating any type of connection.
5. Place Neosporin in cover screw hex prior to suturing over, to prevent the bacterial "bubble" post surgery. If not lanced, this bubble can cause loss of implant.
6. CHx at .125% wont help in the implantitis detox. you need 10-40% Citric Acid, a water laser and or TCN paste to detox surface. Many years ago I heard a British OMS use 3% CHx for 10 min. in a retreatment site to decontaminate the bone defect. This was when an implant was lost post surgery and a large grafting was to follow.
7. CHx is a good pre op ant microbial but can hinder bone graft healing due to fibroblast inhibition, so not good for post surgical wound. Use hot salt H2O rinses per Dr. Silverstein.
