Zygomatic dental implants: training and considerations?
I have been looking into offering zygomatic implants as an option in my practice. There appear to be indications for treatment with these from time to time. However, should these implants be installed only by oral maxillofacial surgeons because of the difficulty and complexity involved or could a GP get the necessary training? Can anyone recommend an intensive hands-on training course for zygomatic implants? Are the risk factors significantly greater with these implants compared to standard implants?

The best training for zygomatic implant surgery is given by Dr. Yvan Fortin in Montreal. Nobel sends all of their specialists there for training, even though Dr. Fortin is a GP. It is an outstanding program given by a world class clinician who has also developed the Marius bridge protocol, specifically designed for reconstruction of zygomatic cases. I have personally taken his program and can attest to his superb skill and knowledge in this complex modality.

I would highly recommend a course given by Dr Greg Boyes Valey (Oral maxillofacial surgeon) and Prof Dale Howes (Prosthodontist) - both South African - refer to the Branemark Institute Johannesburg for further course details.

I would advise only adding this technique if you are very,very experienced. I checked out the first website and the posted cases could have also be handled with a small sinus lift in the molar areas. I do agree with Bedrossians philosophy but even he and other experienced surgeons use zygomatic implants as a tertiary technique it is the last bridge to burn. There are a myriad of complications that can happen and as a surgeon one needs to be able to manage them. The prosthetics need to be exact as they do with an all on four. The experienced gained from orthognathic surgery and zmc fractures is beneficial in this technique and it will take a GP some time to get up to speed on this it is just the way it is. A weekend course with only implant background will lengthen the leaning curve. Be cautious I'm not sure that a dental license will qualify you to perform surgery on the zygoma and a course. Also consider where you practice that patients in the us are litigious. I'm mot saying you can't add this just giving advice, good luck and thanks for reading.

I would life to give some suggestion regarding implants training. You can refer Dr. Mark A. Venincasa for real time exposure for this. He has hand on experience dealing with dental implnats, cosmetic dentistry, dentures, porcelain veneers and many more. He has also awarded Top Dentist in Texas for Winter 2013. Go for live discussion and get a good solution.

I have placed several zygomatic implants, but none in the last 8 years. These implants are impossible to clean and can be avoided in most cases, with the exception of the obliteration defect, left after tumor surgery or trauma. You will find that obtaining anchorage and good tongue values is challenging at best. Some of our colleagues are loading these immediatly. This an awful idea. Bvinci

I will add to this axiom...."if Robert J.Miller recommends Dr. Yvon Fortin, then this is the best piece of information you will need"........ I do not personally do zygomatic implants, but I think I will contact Dr. Fortin to learn more about this technique.

I agree CRS, zygomatic implants are really not something one can really learn in a weekend course . A complication would be hard to defend. Bvinci
